Clinicopathological Characteristics and Treatment Outcomes of Breast Cancer Patients Attending National Cancer Institute − University of Gezira, Sudan during 2012

Breast cancer is the most commonly diagnosed cancer in women worldwide and the most frequent cause of cancer death in women in low-income countries, the breast cancer incidence in Africa continues to increase and is projected to double by 2050. This study aimed to describe the clinicopathological characteristics and determine treatment outcomes of breast cancer in Sudanese women. This was a retrospective one in which a total of 225 women were enrolled, medical records for women treated at National Cancer Institute-University of Gezira (NCI-UG) during the period of January to December 2012 were reviewed. 5-years survival (overall survival and relapse free survival) were determined, and the association of demographic, clinicophathological characteristics and treatment provided with breast cancer outcome were investigated. The median follow-up duration was 59.8 months. The median age at presentation was 45 years (range, 22 to 85 years). 128 (56.9 %) women were from rural area. About two thirds of patients presented with stage III and IV. Invasive ductal carcinoma was the most frequent histopathological type of tumors (196, 87.1%), and about half of investigated women had grade 2 cancers (111, 53.8%). Among women who had investigations for expression of hormonal receptor, 94 (49.7%) women were hormonal receptor positive. Mastectomy was the most common type of surgery performed in 113 (50.2%) of the women. Adjuvant chemotherapy was given to 140 (62.2%) women, 47 (20.9%) women received neoadjuvant chemotherapy, and palliative CT was prescribed to 33 (14.7%) women. The 5-years overall survival rate was 58% and relapse free survival rate was found to be 64.5%. The median progression time in women with stage IV was 16.8 months. Disease stage was significantly associated with overall survival. In conclusion, the survival outcome in Sudanese women with breast cancer is worse than in their counterparts in the developing world, but consistent with results from other African countries. This results call for immediate steps to raise awareness about importance of early detection and to introduce breast cancer screening programs.
