Molecular Characterization of Some Surgical Site Infection in Patients

الملخص

Surgical Site Infection (SSI) has been difficult to manage due to antibiotic resistance of isolates which caused this infection. The present study suggested the isolation and identification of SSI causative bacteria and their antibiotic and disinfectant resistance. Out of 80 clinical specimens and 80 hospital environment specimens collected, 77 (96.25%) and 43 (53.75%), respectively, gave positive culture results, while 3 (3.75%) and 37 (46.25%) gave negative culture results. The numbers of isolates were 53 and 39 isolates, respectively. The most commonly isolated pathogens with pure cultures were Staphylococcus aureus 21 isolates (27.27%), 10 isolates (23.25%), and Escherichia coli 14 isolates (18.18%), 8 isolates (18.60%), followed by Pseudomonas aeruginosa 5 isolates (6.4%), 14 (32.55%); Klebsiella pneumoniae 8 isolates (10.38%), 2 (4.65%); Enterococcus faecalis 5 isolates (6.4%), 5 isolates (11.62%); mixed cultures 24 (31.16%), 4 (9.30%), respectively. The results of phenotypic detection of biofilm formation in clinical and hospital isolates showed that all isolates formed biofilms with a high percentage, while the results of genotypic detection for E. coli and K. pneumoniae showed biofilm formation with a high percentage. The phenotypic detection results of antimicrobial susceptibility tests to commonly tested antibiotics were as follows: In S. aureus isolates, the highest resistance percentage was to Ampicillin (100%), while the lowest resistance percentages were to Vancomycin and Nitrofurantoin (20%). In E. coli, the highest resistance percentages were to Ampicillin, Amoxicillin/clavulanic acid, and Tetracycline (100%), while the lowest resistance percentages were to Imipenem and Meropenem (0%). In P. aeruginosa, the highest resistance percentage was to Amoxicillin/clavulanic acid (100%), while the lowest resistance percentages were to Gentamicin and Amikacin (0%). In K. pneumoniae, the highest resistance percentages were to Ampicillin and Amoxicillin/clavulanic acid (100%), while the lowest resistance percentages were to Imipenem, Meropenem, and Amikacin (0%). In E. faecalis, the highest resistance percentage was to Ampicillin (100%), while the lowest resistance percentage was to Ciprofloxacin (0%). The genotypic detection results of antimicrobial susceptibility of clinical and hospital isolates were as follows: In S. aureus: 10 (47.6%), 4 (40%), 19 (90.4%), 7 (70%) for blaCTX-M-I, parC. In E. faecalis: 2 (40%), 1 (20%), 4 (80%), 3 (60%) for blaCTX-I, parC. In E. coli: [7 (50%), 8 (100%), 11 (78.5%), 6 (75%), 0 (0%), 0 (0%), 12 (85.7%), 7 (78.5%), 12 (85.7%), 6 (75%)] for blaCTX-M-I, aac(3)IIa, ant(2")Ia, acrD, parC. In K. pneumoniae: [4 (50%), 2 (100%), 6 (75%), 1 (50%), 2 (25%), 0 (0%), 6 (75%), 2 (100%), 6 (75%), 2 (100%)] for blaCTX-M1, aac(3)IIa, ant(2")Ia, acrD, parC. In P. aeruginosa: [4 (80%), 14 (100%), 3 (60%), 11 (78.5%), 0 (0%), 0 (0%), 3 (60%), 12 (85.7%), 5 (100%), 7 (50%)] for blaCTX-M1, aac(3)IIa, ant(2")Ia, acrD, parC, respectively. The genotypic results of bacterial isolates for disinfectant resistance of clinical and hospital isolates showed that: In S. aureus, percentage resistance of genes (qacA, SugE) were [(57.1%), (30%), (85.7%), (100%)]. In E. faecalis, percentage resistance of genes (qacA, SugE) were [(40%), (20%), (100%), (100%)]. In E. coli, percentage resistance of genes (qacΔE1, qacE, CepA, SugE, mdfA, ydgF) were [(100%), (100%), (64.2%), (50%), (71.4%), (25%), (100%), (100%), (100%), (100%), (100%), (87.5%)]. In K. pneumoniae, percentage resistance of genes (qacΔE1, qacE, CepA, SugE, mdfA, ydgF) were [(75%), (100%), (100%), (100%), (100%), (100%), (100%), (100%), (75%), (50%)]. In P. aeruginosa, percentage resistance of genes (qacΔE1, qacE, CepA, SugE) were [(100%), (100%), (100%), (100%), (100%), (78.5%), (100%), (100%)], respectively.
