Objectives:Statistical study performed to demonstrate cases of patients with Management of Cerebral Abscess Due to Gunshot Wounds visiting Aleppo University Hospital, and the methods used in diagnosis and management in order to obtain useful recommendations regarding the best surgical treatment and reduces the rate of complications and secondary lesions related to this type of disease, which has good effects on total outcome and leads to the best prognosis and abetter quality of life for the patient and his/her family.Methods:This study has been performed in Neurosurgery Department at Aleppo University Hospital during the period between 2013 –2015 on 22 patients with Management of Cerebral Abscess Due to Gunshot Wound streated with the appropriate surgical intervention.Results:The number of patients involved in this study was (22): 19male (86.4%) and 3 female (13.6%).The most common age group was those who are in the 3rddecades, and the mean age was 24.7 years .Hemiparesis was the most common clinical symptom (63.4%).Glasgow Coma Scale was (13 –15 / 15) in the majority of the admitted patients (77%).Complications were found in 14 patients (27.5%): Lateseizure was the most common complication (68%), followed by meningitis(54.4%).Dilated pupils (which return to normal after urgent surgery) was seen in13% of patients. Hemiparesis was improved in majority of patients after surgery.Parietal region was the most common location of the abscess (40.9%), followed by the frontal region (36.4%) (prognosis was better in frontal abscesses).abscess size was less than 4 cm in (59.1%)of patients.Surgical management include tonsillectomy accompanied by irrigation with warm normal saline and excision of necrotic tissues and foreign bodies, and then performing 69 sub-aponeurotic drainage in (86.4%) of patients, while the brain abscess was aspirated in (13.6%) of patients (abscess size less than 4 cm and didn't contain foreign bodies).Complications was found in (14) patients (63.6%): delayed seizure (after the first week) was the most common complication (68.2%),followed by meningitis (54%).The most important prognostic factors were: location of abscess, gender, and pre-operative (GCS) (prognosis was better in males compared to females).Partial disability was more severe in parietal abscesses.The study showed that there is no relationship between the presence of foreign bodies or extension of fracture line to the orbital cavity or air sinuses and the period between the primary injury and the clinical diagnosis.The study showed that the method of surgical management affect the prognosis,Partial disability was only saw in craniectomy. Conclusion:When patients with brain abscesses due to gunshot wounds were studied, we found that there is relationship between the method of surgical management and prognosis, and the most important prognostic factors were: pre-operative (GCS), location of abscess,size of abscess, and method of surgical management
