Tymanic memebrane perforationisone of the most common issues which facesthe ENT –HNSdoctors in their clinics.The patient usually should undergo a Myringoplasty to close the perforation usinggraft.Tympanosclerosis plaques in the Tympanic membrane or tympanic cavity are one of the challenges which could affect the operation outcomethat’s why we aimed to compare the Myringoplasty results between the patients with andwithout tympanosclersis useing a fascia graft with undelay techniquein addition to study the influence of Tymanosclerosis plaque removal or no removalon the Myringoplasty outcomeMaterials and Methods:Retrospective study in the ENT –HNS department /Almowasat University hospital between 2012 and 2015 on 38 patientsin each group (Cases group) who hadTympanosclerosis plaques on Tympanic membrane orin the tympanic cavity or both(Control group) the patients who had no tympansclerosis plaque.After that we picked up the patientswho had Tympanosclerosis plaques on Tympanic membrane only (26 patients) and we have dividedtheminto twoequalgroups (group A which we removed the plaques during surgery) and (group B which we did not removethe plaques)each group included 13 patientsAll patientshave achieved our study criteriaAll patients have undergone underlay Myringoplasty using fascia graftWehave studied many variablesin addition to compare the Myringoplasty outcomebetween cases and controls and between groupA and B All patients were observedfor 3 months after surgeryAll patientshave been conducted a PTA prior thesurgery and it was repeatedonly for the patientswho achieved graft uptake after 3 months of surgery Average of Air bone gab on (500hz–1000hz–2000hz–4000hz) less or equalto20 decibelconsidered as a clueof hearing improvementTwo tailed Chi square test and Mid P exact testusedas statistical testsaccording to P value (P<0.05 considered statisticallysignificantwith 95% confidence and a=0.05)Results:Graft uptake: (Cases=86.84% Controls=89.47% P=0.73, group A=92.30% group B=76.92% P=0.34)Hearing improvement: (Cases=66.66% Controls=73.52% P=0.53 group A=75.00% group B= 80.00% P=.08)Conclusion:There was a differencebut not statistically significant in the Myringoplasty outcome between Cases and Controls and between Group A and B according to our study criteria
