الملخص
Les méningiomes intracrâniens sont des tumeurs extra parenchymateuses habituellement d’histologie bénigne qui se développent à partir des cellules arachnoïdiennes des enveloppes méningées. Ils représentent 15% de toutes les tumeurs intracrâniennes et sont de siège supra tentoriel dans 90% des cas. Notre étude est rétrospective réalisée sur une période de 6 ans (du janvier 2004 au décembre 2009), portant sur 97 cas de méningiome intracrânien explorés par TDM cérébrale dans 52 cas et IRM cérébrale dans 53 cas, tous confirmés histologiquement. L’âge moyen de nos patients était de 50 ans avec des extrêmes allant de 18 à 80 ans avec une nette prédominance féminine. Les méningiomes étaient situés dans la convexité dans 34% des cas, à la base du crâne dans 26% des cas, à l’arête sphénoïdale et le sinus caverneux dans 21% des cas et le reste est réparti dans d’autres localisations. La TDM et surtout l’IRM ont une place prépondérante et permettent de poser le diagnostic positif et topographique, faire un bilan lésionnel et assurer le suivi post thérapeutique. L’imagerie par résonance magnétique évalue le méningiome en étudiant sa topographie, sa morphologie et son signal. La tomodensitométrie fournit également des arguments en faveur du diagnostic et permet mieux une étude osseuse. Les données de l’imagerie confrontées aux données cliniques permettent souvent une orientation diagnostique précise mais la certitude diagnostique reste histologique.

- Intracranial Meningiomas are extra parenchymal tumors usually of a benign histology that develop from arachnoid cells of meningeal envelopes. They represent 15% of all intracranial tumors and in 90%, the meningiomas are tentorially located. Our study is retrospective applied throughout a period of six years (from January 2004 to December 2009) on 97 cases of intracranial meningioma investigated by cerebral computed tomography (CT) in 52 cases and cerebral MRI in 53 cases, all confirmed histologically. The average age of our patients was 50 years with extremes ranging from 18 to 80 years with a marked female predominance. meningiomas were located in the convexity in 34% of cases at the skull base in 26% of cases, the sphenoidal ridge and hollow sinus in 21% and the rest is distributed to other locations. CT and especially MRI are very important and allow to make the positive and topographic diagnosis, to perform a lesion assessment and monitor post treatment. MRI assesses the meningioma by studying its topography, its morphology and its signal. Computed tomography also provides arguments for the diagnosis and allows to have a better bone study. The data of the MRI compared to the clinical data often permit to have a precise diagnostic orientation, however the diagnostic certainty remains histological.
