- This study was conducted for the period fromthe 1stofSeptember 2011 to 20th ofJuly 2012 to investigatethe infection rateof resistant tuberculosis in Diyala Provinceinsample of150 TB.patientswhichincluded 210sputumsamplsand 45 of washing bronchitis.The result of directmicroscopicalexaminationof sputumusingtheZhellNelsonstainwerepositivein43patientsamples, all sampleswerecultured(positive and negative)on sold Lowenstein Jensenmedia, culturewerepositiveinfiftypatientsamples. Allisolates weretested on biochemical testsand the results showed all isolations were Mycobacterium tuberculosis.Results showed asignificant differencein thenumberoftuberculosiscasesin the age groups(21-30)years. The resultsalsoshowed asignificantdifferenceinnumber ofTB.infectioninmales compared tofemales(21.3%,12%).Results showed asignificant differencein the number oftuberculosisinfectioninsuspectandrelapse(12%)compared withother clinical caseschronic, treadand follow-up(8%, 1.3%and 0%), respectively.Theresults showed that therewassignificant differencesin the number ofTBinfectionin the Baqubasector(21.4%)compared to other sector.The sensitivity testforthefirstlinedrugs showed ahighly significantdifferencesatin thenumberofM.TB.isolatesresistant(MDR-TB.) was40% andthepercentageofmonoresistantisolates 12%and the rate of polyresistant isolates 2%. The highestresistancewas aganestIRSgroup(20%)followed byIPRS, IEPRS, IERSgroups(14%, 4% and 2%)respectively.The results showedhighlysignificant differenceat the level(P<0.01)oftheresistantisolatestoeachantituberculosis drugof first linethe highestresistancetorifampicin (46%), followed by isoniazid, streptomycin, EthambetolandPyrazenamaid(44%,42%, 18% and 6%) respectively.Resultsshowedsignificant differenceatthelevel(P<0.05) in the rateofMDR-TB.by age groupsastheage group(21-30) hashighest ratesofpulmonaryTB(16%) and thelowerratesofmulti-drug resistance were recorded inage groups(61-70) and(71-80) (0.6%).The reselts also shownosignificant differencesinthe rateofMDR-TBin males and females22%and18%respectively. Baqubasector has highestMDR-TB.(18%), inrelapsepatient(28%)compared tonew cases andchronicpatients (8%and 4%)respectively.The sensitivity testM.TB. isolatestosecondlinedrugs, the results ofthe showed noXDR-TB.,2isolatesPre-XDR (10%), and5 isolateswerepoly resistant(25%),and 2isolatesmonoresistant(10%).There wassignificant differencein the proportions ofresistance toantibiotics ofsecondline drugwith higherrate ofresistanceinthe D-CycloserenandRafabutinantibiotics(35% and 30%)respectively,whilethe lowest percentageresistancewas toKnamaycinandP-Amino Salicylicwith rate(10%)for boththeantibiotics. Allisolatesshowed noresistancetoAmikacinandCiprofloxacin.Patient relapsingcasespatientshowedthe highest rates ofresistancetoantibiotics of second line drug7isolatesshowedresistancetoantibiotics of second line drugwith(35%), 1isolatewaspre-XDRwith (5%)and 4isolation werepoly resistantto second line drug with (20%),and 2isolates weremonoresistant(10%), while the number ofsensitiveisolationsto second linedrugfromthe totalisolationsofMDR-TB.are7(35%). Theresultsalsoshowedtherewasa1isolatepre-XDRfrom4isolatesof MDR-TB.Patientwithnew cases(5%),and1poly resistantisolate, 2isolatesofMDR-TB.Chronicpatients(5%)from the totalofMDR-TB.isolates.
